Consent By providing your mobile phone number to your healthcare provider and agreeing to receive text messages, you consent to receive SMS messages from this program. Consent is not a condition of receiving medical care.
Message Frequency Message frequency varies based on your provider's outreach schedule. You may receive periodic reminders about preventive care services, wellness visits, and health assessments.
Message and Data Rates Standard message and data rates may apply depending on your mobile carrier and plan.
Opt-Out Reply STOP at any time to unsubscribe from all SMS messages. After opting out, you will receive one final confirmation message. No further messages will be sent.
