Precision Healthcare Technologies — My Care Message™ Effective Date: September 7, 2026 - Properties of Precision HC Trust, LLC


Overview Precision Healthcare Technologies ("Company") is committed to protecting the privacy of individuals who receive SMS communications through the My Care Message™ platform on behalf of licensed healthcare providers.


 No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service is permitted. 


Information We Collect We collect mobile phone numbers provided directly by patients to their healthcare provider during registration or an office visit. We do not collect this information without your knowledge or consent.


How We Use Your Information Your mobile phone number is used solely to send health-related text messages on behalf of your healthcare provider, including:

  • Annual Wellness Visit reminders
  • Health Risk Assessment (HRA) notifications
  • Preventive care benefit alerts

SMS Messaging By providing your phone number to your healthcare provider and consenting to receive text messages, you agree to receive SMS communications from Precision Healthcare Technologies on their behalf. Message and data rates may apply.


Consent Not Required for Service - You are not required to consent to receive SMS messages as a condition of receiving medical care, treatment, or any services from your healthcare provider. Consent is entirely voluntary.


How We Collect Your Mobile Number - Patients provide their mobile phone number and give explicit written consent at the time of office registration or check-in, or via the online enrollment form at https://precisionvbm.com/enroll-now. No mobile number is collected or used without the patient's explicit authorization.


Opt-Out You may opt out of SMS communications at any time by replying STOP to any message. You may reply HELP for assistance.


Data Sharing We do not sell, rent, or share your mobile phone number with any third party. Information may be shared only as required by law or as necessary to deliver your healthcare provider's messages.
