If you have seen one practice, you have seen one practice. Absolutely none are the same, nor is their patient demographic or individual practice and patient needs. Whether a single provider, provider group, hospital, ACO, etc., each individual provider needs to be onboarded based on the service gaps they have, priorities of the practice, and bandwidth to perform these needed or desired services. Precision Provider Services is that personal matchmaker.
This is the group responsible for providing remote staffing and other services for the provider. Leveraging the PHMS, this group works with providers to plan, implement, and evaluate the various tools and resources needed to successfully deliver care to the provider’s patients. Working under the guidance of the Precision Network Solutions group, they manage the day-to-day interaction and delivery of services to the providers patients.
Services can include Annual Wellness Visits, Chronic Care Management, Remote Patient Monitoring, Transitional Care Management, and others. Critically, the approach used is based on building a true partnership with the clinic with accommodation based on industry best practice, client workflow, reimbursement model(s) and ensuring quality results through shared accountability. Just as important, PPS strives to meet the client where they are and develop a program that augments current program(s) while meeting the client’s definition of success … not just a vendor, but a strategic partner who feels as strongly about meeting client needs as the client does!
